Calculate the amount you owe. This amount is the registration fee of $40 ($20 for students), plus $10 times the number of people for whom you are paying attending the pizza supper plus $30 times the number of persons for whom you are paying attending the banquet, plus the fee for lodging.

The lodging fee is calculated as follows: if you have a grant, you should pay the full cost of sharing a double room ($41 per night) or a single room ($82 per night). If you do not have a grant, and are planning to share a double room, you owe nothing. If you do not have a grant, and are requesting a single room, you should pay $41/night (the difference between a single and half a double).

Check Amount: $ (suggested format: nnn, i.e., no cents.)

If you are a US Citizen and are elegible for support, please include your Social Security Number and your home address.

Please do not send checks drawn on non-U.S. Banks, as we have to pay substantial fee to cash foreign checks. Non-U.S. participants should be prepared to pay the registration and banquet fees in U.S. dollars upon arrival.

Note that registration is not complete until check sent by snail mail is received. The organizers hope to be able to pay the lodging expenses of participants who do not have grants. However, if attendance is greater than expected, participants may have to pay a small additional fee on arrival. Only those who have registered and whose checks have been received by March 7 are eligible for support.
